Home Care Packages (HCP) with Life Care
Home Care Packages are designed to give you choice and flexibility in the way that your care and services are provided. Life Care works with you to create a care plan tailored to your needs and preferences, within your package budget.
HCPs provide government-subsidised care services for older Australians with moderate to high support needs, enabling you to live independently in your own home for as long as possible.
Home Care Package Levels
There are four levels of Home Care Packages to meet different care needs:

Level 1 – Basic Care Needs:
Support for basic needs to maintain independence at home.
Services may typically include basic assistance with tasks like cleaning or meal preparation.

Level 2 – Low Care Needs:
Assistance with daily tasks, personal care, and social support.
Services may include help with personal care, shopping, and transport.

Level 3 – Intermediate Care Needs:
Regular and ongoing support for more complex care requirements.
Services may include more intensive personal care, nursing support, and allied health services.

Level 4 – High Care Needs:
Comprehensive and intensive support for high care needs.
Services are for individuals with complex medical needs requiring a high level of continuous care.
Transitioning to Support at Home
As the Home Care Package program is being replaced by the Support at Home program from 1st November 2025, Life Care will support you through this transition.

You will receive communication from Life Care regarding these changes, including a new Service Agreement.

Your current funding level will stay the same.

There is a “no-worse-off” arrangement, meaning your contributing payments will stay the same.

Your Case Manager will discuss any changes to service lists and price lists with you. Price lists under Support at Home will include all administrative and management costs within the service price.

Your current care plan and services will continue, and if your needs change, a reassessment can be arranged.
Services Available with Your Life Care HCP
Life Care offers a wide array of services under your Home Care Package, tailored to your individual care plan. These can include:

Personal care (showering, dressing)

Allied health services (physiotherapy, occupational therapy, etc.)

Managing Your Home Care Package Funding with Life Care
Life Care provides transparent management of your Home Care Package funds:

Personalised Budget: We work with you to develop a budget based on your package level and care needs.

Monthly Statements: You receive clear, itemised statements showing funds received, services accessed, and any unspent amounts.

Care Management: Our Care Managers assist with coordinating your services, liaising with health professionals, and ensuring your care plan continues to meet your needs.

Package Management: This covers the ongoing administrative tasks required to manage your package.
Home Care Package Costs and Contributions

Government Subsidy: The Australian Government pays a subsidy for your HCP directly to Life Care.

Income-Tested Care Fee: Depending on your income assessment by Services Australia, you may need to pay an income-tested care fee. There are annual and lifetime caps on this fee.

Additional Fees: You can choose to pay for additional services not covered by your package budget on a Fee for Service basis.
