Support at Home (SAH) Program: The Support at Home program is the new model for in-home aged care services, set to replace Home Care Packages from 1st November 2025. It aims to provide a simpler, more flexible, and personalised system.

Home Care Packages (HCP): The Home Care Packages is the current My Aged Care model till 31st October 2025 that provides government-subsidised funding for a range of services to support you to live independently in your own home. Services can include personal care, domestic assistance, home maintenance, and social support.

Home Care Packages (HCPs) provide government-subsidised care for moderate to high support needs. From 1 November 2025, HCPs will be replaced by the Support at Home program.
Support at Home is the new aged care program starting 1 November 2025, replacing HCPs and several other programs. It features eight support levels and new funding categories. The program is designed to be more flexible, with a focus on wellness, reablement, and tailored care to support independence.
Access is via the Single Assessment System. A national workforce uses a Single Assessment Tool, replacing separate ACAT and RAS assessments. This determines eligibility for CHSP, HCP, or the Support at Home program.
Life Care offers comprehensive case/care management, including service coordination, budget management, and regular statements for transparency. Under Support at Home, care management will be an itemised service charged as needed.
Yes! We develop personalised care plans reflecting your goals and preferences. Services must align with your assessed care needs under the Single Assessment System, ensuring compliance with funding guidelines. Under Support at Home, this collaborative planning will strongly focus on your individual goals for wellness and independence.
