Home Care App

Exclusively for our Home Care clients and their families, our App is the easiest way to stay in the loop with your care team. You can jump on anytime to see exactly who's coming over-complete with photos and fun facts about your workers-and what they are coming to help with. Everything you and your family need is right at your fingertips!

Getting Started

Download the App: Search for “Lookout” in the Apple App Store or Google Play Store. You can also access it via your web browser.

Activate Your Account: Look for an “Activate your profile” email from your provider.

  • Can’t find it? Check your junk folder or contact your Care Team to resend the invite.

Log In: Open the app, enter your email, and use the password you created during activation.

Managing Your Schedule

The Schedule tab is your live calendar. It gives you peace of mind by showing exactly who is coming and when.

  • Checked-in: Your Support Worker has arrived and is currently with you.
  • Complete: The visit is finished (includes the exact duration).
  • Vacant: A visit is scheduled, and we are currently assigning the best person to fill it.
  • Visit Details: Tap any visit to see the specific services provided, contact your Support Worker directly, or rate the visit.

Finances & Membership

Tap your Avatar (top-left corner) to access your account details:

  • Account (Statements & Transactions): View your current balance, download monthly statements, or review weekly billing history.
  • Membership: See your funding details, package management fees, and digital copies of your agreements.
  • Help Plan: This is your roadmap to achieving your goals. It’s a live document that outlines exactly how we support you.
